He decided to do himself in.  I swear, it's always the most creative people who do this.  Heck, with all the drugs he'd done in the past it's almost as though he wanted to die back when he was young.  I won't be surprised that the the autopsy shows heroin is his system.  He'd done it before.  He knew that it was an easy (almost pleasant) way to die.  It was not a mistake.
